​Payment

• Payment is due at the time of your session. I accept most major credit cards, debit cards, FSA/HSA cards.  

• At this time, the only insurance accepted is University of Utah Health Care plans.  If you have a University of Utah plan and wish to use it for your sessions, please provide me with your insurance information in advance so that benefits can be verified.  You are responsible for insurance co-payments and any deductible costs.  

• For other private insurance, I can provide you with a superbill that you can submit for reimbursement if your plan offers coverage for out-of-network benefits.  As plans vary, I encourage you to contact your insurance company in advance to understand your coverage.

• I am unable to accept Medicaid or Medicare.

Cancellation Policy

• Life happens and sometimes cancellations are necessary for both clients and me.  There is no charge to you for sessions cancelled 24-hours prior to your appointment time.  Full charges apply for sessions cancelled 24 hours or less.  In the case of sudden illness or an emergency, charges may be waived on a case-by-case basis.  If I need to cancel or reschedule your appointment, I will give you as much notice as possible, reserving the right to cancel for emergency or illness, as life happens for all of us.
