Fees & Policies

​Fees:

• I offer self-pay and insurance billing options.

• Payment is due at the time of service.

  • If you are self-pay or wish to see me as an out-of-network provider, my full fee is due at the time of service.​

  • For services billed to in-network insurance, clients pay their copay and any deductible/co-insurance determined by their policy at the time of service.

• I accept most major credit cards, debit cards, FSA/HSA cards.

​Insurance

• In network with:

  • Blue Cross Blue Shield

  • University of Utah HMHI-BHN

  • Select Health  

• Out-of-network coverage available for many plans.  

​Cancellation Policy

• Life happens and sometimes cancellations are necessary for both clients and me. 

• There is no charge to you for sessions cancelled 24-hours prior to your appointment time. 

• Full charges apply for sessions cancelled 24 hours or less. 

• In the case of sudden illness or an emergency, charges may be waived on a case-by-case basis. 

• If I need to cancel or reschedule your appointment, I will give you as much notice as possible, reserving the right to cancel for emergency or illness, as life happens for all of us.
