​Payment

• Payment is due at the time of your session. I accept most major credit cards, debit cards, FSA/HSA cards.

​Insurance

• In network with University of Utah HMHI-BHN and most Select Health plans.  Copayment, any deductible and/or coinsurance due at the time of service.

• Out-of-network coverage available for many plans.  

• Let's discuss your coverage in advance.

​Cancellation Policy

• Life happens and sometimes cancellations are necessary for both clients and me.  There is no charge to you for sessions cancelled 24-hours prior to your appointment time. 

• Full charges apply for sessions cancelled 24 hours or less. 

• In the case of sudden illness or an emergency, charges may be waived on a case-by-case basis. 

• If I need to cancel or reschedule your appointment, I will give you as much notice as possible, reserving the right to cancel for emergency or illness, as life happens for all of us.
